The legal next of kin must sign all NYC Funeral & Cremation Service paperwork. The legal next of kin must certify that he/she is not aware of any other closer legal next of kin to the Decedent. If the legal next of kin is knowingly misrepresenting himself/herself as the closest legal next of kin to the decedent, the legal next of kin will be responsible for any legal costs that arise from this misrepresentation. NYC Funeral & Cremation Inc. Service has the legal right to sue the legal next of kin that signs paperwork for all the damages and costs due to misrepresentation.
*If the decedent’s weight is 225 pounds or more, there is an additional charge. If the decedent’s weight is 400 pounds or more, we cannot accommodate this, so we are unable to take the case.
You pay for 1 death certificate in the total cremation price. No where does it imply that NYC Funeral & Cremation Service gives a free death certificate. We do provide a beautiful free wood urn.
The name at the top of the death certificate is put in by the doctor, or medical examiner and this cannot be changed by the funeral service. If the Decedent’s name that you provided on the vital statistics form is different then what the doctor put on top of the death certificate, the name that you gave us is put in Box 16 (Aliases or AKA), on death certificates from NYC. On NY state death certificates, AKA is in box 1. If you want the decedent’s name to be changed then please contact the doctor or medical examiner because they are the only ones that can change the name. The Decedent will not be removed until the name has been fixed so this will delay removal.
NYC Death certificates that were completed by a hospital do not include the cause of death.
The next of kin must confirm the death certificate is correct or email any changes within 24 hrs of receiving the work copy of the death certificate from our office. No changes will be made if they do not comply within this time.
The family is responsible for providing 100% correct information for the death certificate and HRA reimbursement forms.
No changes can be made after you choose the cremain delivery method. No changes after the online arrangement form is submitted. All sales final.
Rosemount Memorial Park Crematorium is solely responsible for the cremation of the Decedent, the preparation of the cremains and placement of cremains in the urn, placement of metal tag in urn, preparation of urn labels, certificate of cremation, and Rosemount Memorial Park Crematorium form, and mailing of cremains by USPS with tracking and signature. NYC Funeral & Cremation Service Inc. is not responsible for any delay in mailing of cremains by the US Post Office. Cremains are only mailed to the Legal next of kin.
If Decedent was Medicaid eligible in NYC, we will assist you in getting up to $1700.00 reimbursement
on a cremation. Due to the impact of COVID-19, the HRA allowance has been increased. Full payment must be made by the family to the funeral home. The $1700 reimbursement from HRA is paid directly to the family if Decedent was on Medicaid in NYC.
PLEASE NOTE: The HRA reimbursement is paid directly to the family member that paid for the cremation. We do not accept payment directly from HRA. We do not participate in this program. We will provide all the forms you need for the HRA reimbursement when we mail the death certificate. Extra charges if Decedent is not released to us due to discrepancy in next of kin that was listed on the vital statistics. You will be charged an extra removal fee.
WE DO NOT RE-ORDER DEATH CERTIFICATES. The family goes directly online to www.nyc.gov to order death certificates if the death occurred in NYC (all five boroughs). We do not remove from Hart Island.
After the online contract is signed, if the family decides not to use our cremation service, there is a $250 charge paid by certified check, money order or cash payable directly at the funeral home, or sent by overnight mail.
Please do not schedule a memorial service until you have the cremains of your loved one in your physical possession. NYC Funeral & Cremation Service Inc. is not responsible for any delay in the delivery of cremains to you.
Cremains are mailed separately. Death certificate and HRA Medicaid Reimbursement Forms are mailed only. Tracking numbers for both separate mailings will be provided. We only issue HRA supporting documents one time with the death certificate.
WOOD URN DISCLAIMER: Because wood is a natural product, variations in color and grain are to be expected. Some species of wood have unique characteristics that may cause the final finish to appear darker or lighter in some areas. Our complimentary wood urn might have slight imperfections. There is no return on a free urn.
United States Post Office Disclaimer: We use the United State Postal Service (USPS) to ship urns back to families. In every case, all urns are sent via tracking number and signature required. Although this delivery method is considered a safe method of delivery, there is always a slim chance of package loss. If you are not comfortable with USPS delivery, please pick up your urn directly from the crematory. NYC Funeral & Cremation Service Inc. is not held liable for USPS mailing. Rosemount Memorial Park is held responsible for mailing of cremains.
PLEASE NOTE: We will treat your loved one with dignity and respect, and we expect the families of a decedent to treat our staff with respect. If you are disrespectful to our staff by using inappropriate language and cursing, we will refuse to take on the case and you will not be able to use our service.
